Terms and Conditions

1. We collect personal and sensitive personal information in order to monitor access and implections of S.I.X ( Sex, Information, Xploration) projects for young people (client)

2. When we register you for our S.I.X project through our secure Web site/ paper form, we collect certain personality indentifiable information from you such as you initial, date of birth, sex, postcode, ethnicity, religion and sexual activitiy status.

3. In addition to personally idenifiable information, we collect aggregated nonpersonally identifiable information about activities undertaken by our users.

4. We understand privacy and confidentiality to mean that, apart from in the limited situations below, personal and sensitive information regarding a client shall not be passed or communicated to anyone outside of our service* without the client's prior explicit consent to such disclosure.

5. We are committed  to providing a confidential informtion, advice and guidance service to young people and client information will be only used in accordance with professional practice, the Data Protection Act 1998, and other relevant legislation.

6. We aim to share the information that we collect with our partner boroughs (Lamberth and Southwark) in order to enable easy access for our clients to sexual health information, advice and condom distribution scheme.

7. Each borough will have a coordinator who is responsible for overall coordination of this project who appoints/approves users of website in the borough.

8. Approved website users will have an access only to an individual client's information where the client has consented for the information to be shared or that the information was collected by that partner borough.

9. The coordinators will have internal access to only aggregated data for reporting purposes at each borough level.

10. Boroughs will have no access to each other's data and will be responsible for security and confidentiality of clients information in line with security standards and the Data Protection Act 1998.

11. Clients information will be destroyed once he/she reaches 25 years of age.

12. All staff should ensure that no discussion identifying an individual client take a place outside our premises and keep records of clients secure.

13. All new members of staff will be introduced to the Confidentiality Statement during induction.
